Primary Anaplastic Pleomorphic Xanthoastrocytoma with Rapid Progression

Abstract

|
A 42 year-old man presented with transient motor aphasia and memory impairment for 2 months. Magnetic resonance imaging (MRI) with gadolinium showed a single enhanced mass in the left temporal lobe. The patient underwent gross total tumor resection under awake surgery with the frameless navigation system. Pathological diagnosis was pleomorphic xanthoastrocytoma (PXA) with anaplastic features. Immunohistochemical staining showed 12 mitoses per 10 high power fields and over 50% of the cells were Ki-67 immunoreactive. Postoperatively, local irradiation (60 Gy¡¿40 days) began. Two weeks later, follow-up MRI showed rapid progression of tumor growth and huge perilesional edema. We report a case of primary anaplastic PXA with aggressive behavior despite surgical excision and irradiation.
